United States & Canada:

Expectation: Tipping is strongly expected and forms a significant portion of service workers' income (especially in restaurants).
Restaurants: 15-20% is standard. 18% for good service, 20%+ for excellent. For poor service, 10% or less, often accompanied by speaking with management.
Taxis/Bars/Hotels: 10-15% is common.

Europe (General Trends):

Expectation: Less ingrained than North America. Service charges are often included, or a small tip is for exceptional service.
France: Service charge ("service compris") is almost always included. Locals might round up to the nearest Euro or leave a few Euros for good service, but it's not obligatory beyond the service charge.
Italy: A "coperto" (cover charge) or "servizio" (service charge) may be added. An additional small tip (rounding up or a few Euros) for good service is appreciated but not mandatory.
